The application form can be obtained from DVA offices of the nearest state branches or downloaded from the DVA website (www.jhev.gov.my). Every application must accomplish the Main Form (BQ-BK-01) together with the application form of the scheme applied for.

All applications must be submitted through the designated nearest state DVA branches. The department will evaluate the applications from the state DVA based on applicants justification and recommendations. The applicant will be duly notified of the status of their application.
