Borang Permohonan

Borang permohonan boleh didapati dari Pejabat JHEV Cawangan Negeri yang berdekatan atau boleh dimuat turun dari laman web Jabatan Hal Ehwal ATM (www.jhev.gov.my). Setiap permohonan hendaklah memenuhi Borang Induk (BQ-BK-01) dan diikuti dengan borang skim yang dipohon.
